Medical records

How to ask for a copy of your records. Texas Board of Chiropractic Examiners Rule §76.2 says a request for patient records must be made in writing, signed by the patient or a legally authorized representative, and must say which records are wanted and who should receive them.

1. Send us a written request

In person or by mail: Pure Health & Wellness Chiropractic Center, 750 Schneider Dr., Suite 170, Cibolo, TX 78108.
By email: purehealthchiro1@gmail.com
By fax: (210) 566-8799

2. Include this information

  1. The patient’s full legal name
  2. The patient’s date of birth
  3. A phone number or email address
  4. Which records you want: all chiropractic records, records from certain dates, X-rays or imaging, billing records or treatment notes
  5. Where to send them: to you, another doctor, an attorney, an insurance company or another authorized person or organization
  6. The signature of the patient or their legal representative
  7. The date you signed it

3. We confirm who you are

Before releasing records we may ask for photo ID, or for papers showing that you may request records for the patient. Texas Board of Chiropractic Examiners rules allow the office to check this.

4. Fees

Digital records are free when you agree to receive them electronically. Printed copies: the first 10 pages are free, then $1.00 a page.

5. When you will get them

Within 15 business days of a valid request and any fee, unless an exception applies under Texas law or the Board’s rules.
